Investigation Summary
At approximately 9:00 a.m. on December 7, 2009, Employee #1 was preparing to install copper flashing on a roof. Shortly after 9:00 a.m., the jobsite foreman saw Employee #1 arrive and begin setting up at the northwest corner. At approximately 9:30 a.m., a coworker saw Employee #1 carry a two inch by twelve inch board over to the area. At approximately 10:38 a.m., the jobsite foreman noticed that he had not heard from Employee #1 for awhile and went looking for him. The jobsite foreman found Employee #1 unresponsive on the ground between the bases of two ladders that had been set up to the roof. The jobsite foreman immediately called 911. Employee #1 was transported to Greenwich Hospital, where he was pronounced dead at 11:40 a.m. During the course of the onsite investigation, it was determined that between 9:00 a.m. and 10:38 a.m., the employee had set up two 24-foot extension ladders to the roof and had installed one roof bracket scaffold, consisting of two brackets and a two inch by twelve inch plank that was approximately ten feet long. There were no footprints in the snow on the roof, indicating Employee #1 had not yet begun work on the roof. Since no one was working with the employee and no one observed the accident, it was assumed that Employee #1 fell while working on the ladder at a maximum height of 23 feet. The autopsy results showed the employee had an approximate one inch laceration to the rear of his head; however the cause of death was ruled to have been acute myocardial infarction due to coronary atherosclerosis, which was not apparently related to any type of fall or other apparent workplace condition. Employee #1 was an employee of R.C. Torre Construction Corporation Inc.
