LOGGER STRUCK BY FLYING PIECE OF FALLING TREE

TWO EMPLOYES WERE FELLING TREES. ONE WAS USING THE CHAIN SAW, THE OTHER CARRIED TOOLS AND SUPPLIES. A SMALL TREE WAS WAS FELLED INTO STANDING TIMBER. THE FELLED TREE STRUCK AN OLD LOG WHICH HAD BEEN IN CONTACT WITH ONE END OF A LARGE SNAG. WHEN THE FELLED TREE CAME TO REST, THE TWO EMPLOYEES TURNED THEIR BACKS AND WALKED TO ANOTHER TREE. THE SNAG, WHICH HAD BEEN DISTURBED BY THE FELLED TREE, FELL IN THE DIRECTION OF THE TWO EMPLOYEES. EMPLOYEE #1 WAS STRUCK IN THE HEAD BY A FLYING CHUNK OF THE SNAG AND KILLED. CAUSAL FACTORS: 1. THE TREE WAS FELLED INTO STANDING TIMBER RATHER THAN AN ADJACENT STRIP OF FELLED TIMBER. 2. THE CHAIN SAW MOTOR WAS NOT STOPPED, AS REQUIRED. THIS WOULD HAVE PERMITTED THE EMPLOYEES TO HEAR THE SNAG BEGIN TO FALL. 3. HARD HATS WERE NOT WORN.
