Investigation Summary
On August 7, 2009, Employee #1 climbed up warehouse racking to move a pallet that was wedged into an 18-inch gap in the racking. Employee #1, who was not wearing fall protection, lost his footing and fell through the racking to the concrete floor 10 feet below. Employee #1 apparently sustained a concussion and was hospitalized.
