Employee's Fingers Are Severed by Dock Plate

On August 25, 2009, Employee #1 was unloading a semi-trailer to position the dock plate inside of the trailer. The dock plate was broken and continued to malfunction. The employee could not get the dock plate to lift by the manual handle provided. The employee used a forklift, with the help of a coworker, to help lift the dock plate. The dock plate fell and severed two of his fingers. Employee #1 was hospitalized.
