Investigation Summary
On February 16, 2010, an employee was tying to main water line for new housing being built for the Kickapoo tribal elders. The main water line was 17-20 feet underground. The walls of excavation were straight vertical without protective devices and without sloping/shoring. The reported initial excavation was 17-ft deep and 8-ft by 8-ft square. The walls of the trench collapsed, trapping and killing the employee.
