Investigation Summary

Investigation Nr: 200644581
Event: 12/23/2009
Employee Is Struck and Killed When Run over by Trailer

At approximately 8:30 a.m. on December 23, 2009, Employee #1 and a coworker were unloading piping from the ground and next to a tractor trailer. The pipe yard was located in an industrial area west of Ponca City, OK. The truck driver drove away after the pipe was unloaded because he had a visual "all clear" from one of the workers on the ground. The truck pulled forward and the driver saw the second worker on the ground as the back wheels of the trailer ran over the worker. Employee #1 was transported to Hillcrest Hospital in Tulsa, where he died at approximately 3:30 pm on the same day.
