Investigation Summary
At approximately 8:30 a.m. on August 27, 2009, Employee #1, a production manager, was going to work to clean up the warehouse. He was not seen again that morning. There was a forklift in the warehouse that would have been used to move pallets of door cutouts in the warehouse. Two coworkers went into the warehouse that morning to borrow the forklift and use it for a short time. Coworker #1 was a production worker who used and returned it, and Coworker #2 was the shop foreman who had kept it to complete some loading operations. Neither coworker noticed the toppled stack of door cutouts. Coworker #3, a mechanic, noticed that the employee's truck had not been moved all morning and no one had been able to reach him on the 2-way radio. A preliminary search of the plant had not been able to locate him. On a second search, a Coworker #3 noticed the overturned pallets and found the employee under them. Emergency Medical Services were called and responded. Employee #1 was pronounced dead at the scene by the EMS. The cause of death was crushing and asphyxiation.
