Investigation Summary
At approximately 9:30 am on March 18, 2010, Employee #1, of Fredinburg Masonry Inc., was removing scaffold planks from a scaffold surrounding a chimney stack. Employee #1 stepped onto a slippery 4:12 pitch roof with eave to ground height of approximately 22 feet. He slipped on the roof surface and fell to the ground below. Employee #1 sustained a fracture to his left arm.
