Investigation Summary

At approximately 1:30 p.m. on June 14, 2012, Employee #1 was sweeping the loading dock at Tri Marine Fish Company. As Employee #1 swept the loading dock, coworker hooked up a dock plate from a loading dock to the front end of a fork lift and began to move the fork lift in a backward direction to move the dock plate to another loading dock. As the coworker backed the fork lift up the right side of the dock plate, the fork lift hit Employee #1 in the back, knocking the employee forward and causing her to lose her balance and fall to the concrete ground onto her left hip. The coworker called 911 and the Los Angeles Harbor police division responded to the 911 call with paramedics and transported Employee #1 to Harbor UCLA hospital in Carson, CA. She suffered unspecified injuries and was admitted for treatment her injuries.
