Investigation Summary
At approximately 10:29 p.m. on April 19, 2012, Employee #1, from TS Staffing working at Pacer Distribution Services, was driving a stand-up forklift while he was loading pallets of product into the trailers at the dock. Employee #1 lost control of the forklift. His left thumb was crushed between the back of the lift and a metal guardrail at the Shipping Department desk in the warehouse. Employee #1 had not been trained to drive stand-up forklifts. Employee #1 was treated in a medical center.
