Employee Crushes Fingertip in Machine and Is Amputated

At approximately 9:00 a.m. on September 14, 2011, Employee #1 was working at an automotive facility, where alternators and starters were received, torn down, refurbished and provided for the automotive retail market. He was assigned to the painting recovery department in which starter armatures are disassembled and painted in a process. The process involved a machine identified as the butterfly machine (Model Number GPED3110B, Serial Number A2503C), which secured the armature in place to remove the armature from the bearing. During the activity, Employee #1 placed the armature on the chuck with his left hand and secured it in the chuck jaws. Then, using his right hand, he reached to the left side of the chuck and triggered a press lever. The lever activated the ram that pressed against the armature and removed it from the bearing. While the ram was activated, his left middle finger was caught between the ram and the inner wall of the bearing, which crushed the fingertip. Employee #1 was transported to medical center, where he underwent treatment and was then released from the hospital.
