Investigation Summary
On September 29, 2010 Employee #1 was working with a crew applying slurry seal on asphalt, a process designed to lengthen the life of the material. Applying sealant on the road involves a truck driver of a slurry truck, a slurry operator who stands at the rear of the truck, driver side and one or two personnel on the ground, using a squeegee to spread the slurry evenly on the road, making sure that the slurry covers every inch of the road. On the day of the accident, there were two personnel conducting squeegee operations. The work on that day was on a curved, uphill road. While conducting squeegee operations, Employee #1 noticed that they were so far behind the truck. So he ran towards the truck, and started squeegee operations directly behind it while walking backwards. As he walked backwards, his left foot stepped back into spreader box where it was caught in the auger, causing it to fracture. Employee #1 was hospitalized for several days where he had to undergo surgery.
