Investigation Summary
On the day of the event, Employee #1 was in route to her office walking on a concrete sidewalk pulling an expanding folding crate. Employee #1 was walking on the right side of the walkway, as the crate she was pulling crossed over an irrigation valve box cover. The cart's wheel caught on the plate and Employee #1 fell backwards to the ground on to her right hip. Employee #1 was assisted by a student, campus police and paramedics were contacted. Employee #1 was transported to a local hospital. Employee #1 experienced a fracture to the right hip, and was hospitalized for hip surgery.
