Investigation Summary

Investigation Nr: 202530432
Event: 11/16/2010
Employee's Arm Is Caught in Conveyor and Is Injured

At approximately 4:30 a.m. on November 16, 2010, Employee #1 and four coworkers were working on a recycling plant's second floor platform and in the vicinity of a running unguarded conveyor belt and pulley. Employees were performing housekeeping duties, which included picking up scrap metal that had fallen from the conveyor belt. Employee #1 had his back to the unguarded conveyor belt and pulley. He was using a shovel to clean the area when his clothing was caught by the moving parts of the conveyor. He attempted to free himself, and his left arm was pulled into the conveyor. Coworkers stopped the conveyor via an emergency stop and notified the plant's supervision, who summoned emergency service personnel. The Fire Department responded and freed Employee #1 from the conveyor. They transported him to a medical center. At the medical center, he underwent surgery for injuries that included a compound fracture to his left arm and was then hospitalized.
