Investigation Summary

At approximately 10:30 a.m., on October 23, 2010, Employee #1, a 17 year-old male with Primetime Grill Corp Dba Gerry's Grill, was moving dishes in the dish washing area when he slipped on the wet floor and fell to the ground. After falling to the ground Employee #1 got up and continued working. After Employee #1 finished his shift he rode his bicycle home and went to bed. The next day October 24, 2010, Employee #1's uncle found Employee #1 on the floor of his bedroom not breathing. His uncle summoned Emergency Medical Services. The Los Angeles Police Department along with Los Angeles Fire Services responded to the call and at approximately 9:35 a.m., a representative of the Los Angeles Fire Department pronounced Employee #1 to be deceased at the scene. The Los Angeles County Coroner conducted an autopsy of the body and on the 31st of January 2011, the division received a copy of the Los Angeles County Coroner report. The Los Angeles Coroner was not able to determine if the cause of Employee #1's death was work related.
