Investigation Summary
On June 30, 2009, Employee #1 and coworkers were removing temporary bracing for the concrete barriers on a bridge and installing the permanent wide flange beam bracing. They attached the permanent beam bracing by drilling through the barrier and bolting it into place. This work was accomplished via a scaffold-type assembly that was fed through the bottom openings of the concrete barriers (scupper holes) and then dropped below the bridge deck. Employee #1 attempted to access the scaffolding to check to see if the holes being drilled were in the proper angle. He leaped down approximately 5 ft onto the scaffold platform and landed on the far side of the scaffold plank. When he landed on the plank, the plank cantilevered and both he and the plank fell approximately 15 ft onto concrete. He then slid down the concrete embankment into the water and was rescued by employees that were working above. He sustained unspecified injuries. Employee #1 was transported to a medical center, where he underwent treatment and surgery, and was then hospitalized.
