Investigation Summary
On July 7, 2009, Employee #1 was cleaning the chrome hold down of a uncoiler machine. His hand was positioned on the uncoiler plate, when the hold down came down and pinched his fingers. He was hospitalized with amputated middle, ring and pinky fingers.
