Investigation Summary
On September 15, 2009, Employee #1, a parcel courier, left her delivery truck and approached the front end loader at a sand pit. The courier tried to get the attention of the front end loader operator but failed. The front end loader operator noticed the delivery truck and backed over to the road to accept the package. When he got to the road he looked forward and realized he had run over Employee #1. Employee #1 died.
