Employee's Foot Is Struck by Forklift and Sustains Injuries

At approximately 11:30 p.m. on November 14, 2011, Employee #1 was walking along the right side of a swing-reach forklift truck and talking with the turret operator. The forklift was a Raymond turret lift truck (Model Number 970-CSR 30 T). Employee #1 wanted to be lifted to an elevated location, so he leaned on the right arm of the turret and had his left foot on the edge of the turret platform. The forklift operator started lifting him, when he became anxious and requested the operator to lower him. As he was lowered, Employee #1 slipped and his left foot inadvertently was struck by the forklift's right front wheel. He sustained serious crushing injuries of his left three toes (third, fourth and fifth digits). Employee #1 was transported to a medical center, where he underwent procedures to stabilize the fractures and was hospitalized to receive postoperative care.
