Investigation Summary
At 11:00 a.m. on September 6, 2011, Employee #1, an equipment operator employed by a general engineering contractor that engaged in laying underground pipes for utilities. Employee # 1 changed the compaction wheel of a "Caterpillar, model #446B, serial #2FG07852" backhoe loader/ excavator after digging several 4 to 5 feet deep trenches at the site. Employee # 1 was walking around the backhoe when he stepped on a wet spot with some dried mud hitting his left knee on the ground snapping his left upper leg. Employee # 1 was taken to Riverside Community Hospital, Riverside, CA. He was hospitalized for three days with multiple injuries, including a fractured femur. The accident was reported to DOSH (Division of Occupational Safety and Health) on September 7, 2011 by the employer.
