Investigation Summary
At about 7:00 p.m. on September 6, 2011, Employee #1, a temporary worker at a plastic container manufacturer, was clearing jammed material in a Pagani grinder (Model Number 2535.FA, Serial Number 2535.98.1574, RPM 1755, C.P. 15, Employer Identification Number 7). He opened the hopper portion and removed the machine's front cover to access the jammed material in the grinding chamber. The grinder blade had stopped due to the jam in the chamber. While pushing the material with his right hand from the top of the grinder chamber and pulling the material with his left hand from the front, the grinder blade activated and pulled Employee #1's left hand in. He was hospitalized for the amputation of all five left fingers.
