Investigation Summary
At approximately 5:20 am, on September 6, 2011 Employee #1, a day laborer of Rich Diaz, became seriously injured while working at a construction site. On the day of the incident, Employee #1 found that a forklift had been moved from the rear of the facility to the front of the facility. The forks were raised about 2 to 3 feet off of the floor, and there was plywood laid across the forks. Employee #1 proceeded to the electrical panel to turn on the lights. As he did so, Employee #1 took it upon himself to see what was causing the forks to be elevated. He could see that there was some plaster debris in the mast. After freeing some debris, he dismounted the forks. Upon reaching the ground, he realized that he had left his new needle- nosed pliers back up on top of the mast. Employee #1 proceeded to climb back up on the forks and then placed his hand through the mast to collect his pliers. As he did so, the mast gave way and the forks and mast came down, pinning his left forearm in between two mast sections, impaling his left palm with the needle-nosed pliers in the process. A surgical team from Arrowhead Regional Medical Center in Colton, CA was called out, and the surgeons proceeded to remove Employee #1's left hand in a manner that would not result in excessive blood loss. Employee #1 was transported to Arrowhead for postsurgery treatment.
