Employee Slips and Falls While Climbing Ladder

At approximately 7:45 a.m. on August 11, 2011, Employee #1, an experienced maintenance manager, was checking electrical wire connections in the junction box of a 12-volt, Security Alarm system panel that was sounding a false alarm. The junction box was located at an elevation of 10 feet above the concrete floor in close proximity to a fire riser and next to the main entry door of the warehouse. He placed a WERNER 24-ft extension ladder against the wall. The ladder foot rests did not have rubber slip resistance protectors and there were exposed rivets at the base. After positioning the ladder, he began climbing, but slipped and fell, causing injuries to both wrists, both lower arms and a left cheek bone fracture. Employee #1 was taken to Arrowhead Regional Medical Center, where surgery was performed and he was hospitalized for three days. The ladder with defective foot rests was not withdrawn from service for repair or destruction, nor tagged or marked as "Dangerous, Do Not Use".
