Investigation Summary
At approximately 2:30 p.m. on May 31, 2011, an employee was located between the two semi-trailers #1 and #2 parked in parallel in the rental yard. The employee was doing a pretrip test on his semi-trailer #1 at which time the Gradall operator was loading the light tower on semi-trailer #2 from its east side. As the Gradall operator was loading or setting the light tower onto the upper deck of trailer #2, it slipped off the forks and fell, pinning the employee between semi-trailer #1 and the light tower frame. The employee was hospitalized and treated for multiple facial injuries.
