Employee Amputates Forearm While Cleaning Cylinder on Pump

At approximately 3:44 p.m. on May 10, 2011, Employee #1 was using a water hose to clean a Centurion concrete pump, Model Number XR 3070, Serial Number 013104XR3070. He cleaned the left cylinder of the pump and then moved the connecting pipe to the left to clean the right cylinder. As he placed his left arm into the right cylinder, the connecting pipe shifted to right side, amputating Employee #1's left forearm. Employee #1 was taken to a nearby medical facility, where he was treated for his injury.
