Investigation Summary
At approximately 12:13 p.m., on March 16, 2011 Employee #1 was seriously injured while on duty at a California Department of Food & Agriculture Inspection Station. Employee #2 was driving a Toyota Tacoma gray truck (Lic # 8X19098, CA) towing a "Carson Car Trailer" (Blue) through Employee #1's assigned lane. Employee #1 had just given permission to Employee #2 to proceed after completing inspection. As Employee #2 began to accelerate out of the Inspection Station, Employee #1 stepped to the right out of her booth directly into the path of his left trailer wheels. Before Employee #2 could stop, Employee #1 was struck by Employee #2 trailer's left fender knocking Employee #1 to the ground.
