Investigation Summary
At approximately 4:45 p.m. on February 25, 2011, Employee #1 sustained amputation of his left index finger when he was stripping the teflon tube off the steel tube by pounding it manually on top of a larger steel tube placed on the floor. During pounding, Employee #1 hit his left index finger on the larger steel tube causing amputation of his index finger up to the nail.
