Worker Cleaning Near Running Pulley Amputates Finger

At approximately 12:30 a.m. on January 29, 2011, an employee was working as a laborer for a firm that manufactured metal expanders for HVAC filters. He was a leased employee from a temporary help service, and he had worked for this employer regularly as a laborer for 3.5 months prior to the date of accident. He was working near the firm's metal expander machine Number 12, which was a Model Number FLMY EX-30 Precision Air Machine metal expander, with Serial Number 0538. The narrative indicated that the motor was operating at 175 rpm. It is possible that this was a typo and that 1,750 rpm was the motor's speed. OSHA was unable, however, to verify the actual speed of this equipment. He was cleaning the main shaft of a motor and the floor area underneath, in proximity of the running timing belt of the motor, located in the back of the metal expander. As he was cleaning the floor area with a towel or rag he held in his gloved right hand, he inadvertently came into contact with the running timing belt of the motor. His hand was caught and pulled into the running belt and pulley drive, and it was crushed. Following the accident, the index finger on his right hand was completely amputated at a hospital.
