Investigation Summary
At approximately 8:30 a.m. on December 1, 2010, Employee #1 was operating a Marathon horizontal stationary baler (Model Number SE-504842-830). While the baler was running in automatic mode, he threw broken plastic pieces of totes and cardboard pieces into the hopper (located above the platform with 13 in. high collars) for processing. During baling, he noticed that the plastic pieces were not compacting properly. He decided to climb the baler platform from the rear and stepped into the hopper to push the plastic pieces down with his both feet while holding its collar edge with both hands. As he pushed the plastic pieces down, both of his legs contacted the moving ram, which resulted in serious crushing leg injuries. Employee #1 was transported to a medical center. At the medical center, he underwent surgical procedures, which included the amputation of both legs. After the surgery, he received postoperative care and was then hospitalized for six weeks.
