Investigation Summary
At approximately 10:30 a.m. on September 20, 2010, Employee #1 was certifying a truck mounted mobile crane, Maintainer Crane Model Number 5220,with a capacity of 5 tons on a Peterbilt truck, Model Number 330SE, License Plate Number 6D90833. The crane operator was backing up the crane and Employee #1 was giving signal him. Employee #1 told the operator to stop the crane near the test weights. As the operator tried to apply the parking brake, his foot slipped off the clutch pedal and the truck jumped back and struck Employee #1 on the left side of his pelvic area. Employee #1 was pinned between the back of the truck and a test weight causing multiple fractures to his pelvic area. Employee #1 was hospitalized for a fractured pelvis.
