Employee's Toes Are Amputated by Falling Forks

At approximately 7:10 a.m. on July 14, 2010, an employee along with two coworkers was securing a carriage system, weighing 2163 lbs, dimensions 70-in. Wide by 39-in high by 4-in. tall, of a "Hyster, Model Number H180E fork lift to its mast frame for moving it out of the way, because of loss of power; it could not move steering or forks at all. The carriage was lifted up by placing it on one fork of another Hyster, Model Number H210HD fork lift at about 36-in. above the ground. The employee was standing in front of the carriage while he was trying to grab a chain from a second worker who was standing behind it when suddenly the carriage slipped off the fork and fell on his right foot causing the serious injury. He sustained amputation of his right big toe and crushing injury of second toe. The employee was taken to Loma Linda University Medical Center, Loma Linda, CA, for treatment. Surgery was performed and the employee was hospitalized four days.
