Investigation Summary
At approximately 7:30 a.m. on June 12, 2010, After dropping the drop gate, Employee #1 entered the wash up area and proceeded to clean the running magnets with a rag in his left hand. As Employee #1 was cleaning the rotating magnets, suddenly his left gloved hand got caught and pulled in the running side of the composition roller and the steel roller, causing amputation injuries of his fingers.
