Employee Legs Are Injured by Falling Table and Rebar

At approximately 4:30 a.m. on March 26, 2010, Employee #1 had set a small stack of bars on a metal table utilizing a 5-ton overhead crane. There were hooks attached to two chains hanging from the overhead crane. Employee #1 unhooked one of the two chains and was getting ready to unhook the other chain. Employee #1 proceeded to lift the overhead hoist with the hoist controller and the table lifted up. The bars and the table fell onto Employee #1's legs. A coworker tried to stop the bars from falling but the weight was too much. Employee #1 was knocked to the ground and lying flat on the ground. The rebar and metal table fell onto both Employee #1's lower legs. Employee #1 stated that he did not know how much weight was on him. Employee #1 sustained serious injuries to his right and left lower legs and was transported to the San Antonio Medical Center, where he was admitted and surgery was performed. Employee #1 was hospitalized for approximately three days; upon his release from the hospital, he was transferred to a rehabilitation center where he remained for approximately one month. A member of the responding fire department notified the Division on March 26. Employee #1 is an employed at a facility where re-bar is cut to customer specifications.
