Investigation Summary
At approximately 12:30 p.m. on February 15, 2010, Employee #1 was walking across the joist so he could ask for more material to cut. Employee #1 lost his balance and fell onto a block. Employee #1 landed on the left side of his stomach. Employee #1 got up and climbed down the ladder. Employee #1 sustained serious injuries to his abdomen when he fell part way through a floor joist and hung by his arms. A coworker drove Employee #1 to a clinic. Employee #1 was then taken by ambulance to Arrowhead Hospital. Employee #1 was admitted and surgery was performed. Employee #1 was hospitalized for approximately six days. The Division was notified of the injury at approximately 2:46 p.m. on February 16, 2010 by the employer. The District Manager assigned the accident investigation to an investigator on March 8, 2010. The investigator conducted the inspection on April 7, 2010. The site was a condominium complex under construction.
