Investigation Summary
At approximately 11:45 p.m. on October 13, 2009, an employee was cleaning a large spray dryer, Sweco Vibro Energy, Model Number LS248460-3, Serial Number 716187-4199. As he tried to check for debris inside the housing of Airlock Vane Rotor, Mikro Pul, Model Number N-1, Serial Number 71A2193, by using a paper towel with his right hand, the rotating blades struck his right index and middle finger and caused amputation of two fingers. The employee was hospitalized and treated for fingers amputation.
