Investigation Summary
At approximately 11:03 a.m. on September 16, 2009, an employee was making a 0.3125-in.planing step on the edge of a piece of wood, 20.875-in. long by 7.5 in. high by 1.75-in thick, on a Grizzly, Series Industrial Jointer Number 2135, rpm 5000. She was pushing the piece of wood with her both hands, right hand in the back and left hand on the forward middle side of the piece over the jointer table. As she was pushing the piece, suddenly the front bottom edge of the piece stuck at the cutting blade causing the back end of piece to lift up and contacting her left little finger with the running blades. The employee was hospitalized and treated for amputation of her left little finger.
