Investigation Summary
At approximately 6:30 a.m. on October 3, 2009, Employee #1, a laborer, was using an over-seeding machine (Thatch-Master, Model Number TM-720, and Serial Number CM-3135). He did not stop the machine when he attempted to clean off the grass cuttings from the blade of the machine. He sustained a partially severed right foot when his foot made contact with the running blades. Emergency medical services responded to the scene and Employee #1 was transported by ambulance to the Desert Regional Medical Center, where he underwent surgery and was hospitalized for a three days.
