Investigation Summary
At approximately 5:58 a.m. on May 29, 2009, Employee #1, a master mechanic, was performing a diagnostic procedure on a Holset turbocharger, Model Number HE551V, installed on a Cummins diesel engine. The employer operates a trucking operation that is primarily involved with the operation and maintenance of their own tractors and trailers. Employee #1 performs maintenance on the tractors, which includes the service and specific repair of the engines. As Employee #1 was performing the routine, but infrequent, turbocharger diagnostic procedure with the air inlet tube removed from the opening of the turbocharger air intake compressor, he was moving his hands around the engine to detect air leaks. His left hand passed too close to the inlet opening of the operating turbocharger and was drawn into the compressor wheel. One of his fingers contacted the spinning intake impeller blades, resulting in the amputation of the tip of his left ring finger. Employee #1 was taken the Arrowhead Regional Medical Center Emergency Department, where he was treated for his injury.
