Investigation Summary
At approximately 3:40 p.m. on July 23, 2009, Employee #1 was operating a forklift, Number 15, moving a pallet of wooden poles. The load shifted and he used his left hand to prevent the load from falling. His finger was pinched between the frame of the forklift and the load, amputating it.
