Investigation Summary
At 1:30 p.m. on August 18, 2009, Employee #1 was picking up some boxes containing medical supplies for elderly care from a stock of merchandise boxes, located in an EW aisle Number C. The products were stored on the second rack shelf, which was 8-ft deep and elevated 9 ft 2.5 in. above the concrete floor. In order to pick up a box, Employee #1 accessed the shelf by stepping off the work platform of a Raymond Number 20, Model EASi-OPC 30TT order picker. Employee #1's body belt was not hooked to the lanyard and tether. After picking the box, Employee #1 was walking back on the work platform to put the box on the top of already stacked large boxes on the pallet. Inadvertently, he stepped off the edge of the platform and fell 9 ft 2.5 in. to the concrete floor, along with the box. Upon landing on the floor, he sustained serious multiple head trauma and fractured his both wrists and two ribs. Employee #1 was taken for treatment to Arrowhead Regional Medical Center in Colton, California. The surgery was performed the same day, and he was hospitalized for more than four months.
