Investigation Summary
At approximately 7:30 a.m. on June 30, 2009, Employee #1 and a coworker, both roofers, were engaged in removing the old cap sheet on a youth center's roof, which was located in close proximity to a preexisting 8 ft by 8 ft roof opening (atrium). The coworker used a shovel to remove and cut the cap sheet, while Employee #1 pulled and held it to a length of 4 ft to make piles (stacks) of the sheets on the roof. When Employee #1 was attempting pull and hold the cap sheet, he inadvertently fell 13.75 ft through the roof opening and onto a concrete floor. He sustained serious head trauma and rib fracture. Employee #1 was transported to a medical center, where he underwent treatment and surgery, and was then hospitalized.
