Employee Is Injured in Fall from Elevated Container

At approximately 1:45 p.m. on November 5, 2009, Employee #1 was standing in a container that measured 52-inch by 48-inch, shoveling ammonia sulfate from the container into a truck. As the forklift operator lowered the container, it slipped forward and fell off of the forklift forks, 4-ft from the ground, with Employee #1 inside the container. Employee #1 sustained a laceration to his eye from the shovel that he was using, and hit his head and right arm. He was transported to Salinas Valley Memorial Hospital but was not hospitalized for treatment.
