Investigation Summary
On June 17, 2009, Employee #1 was clearing an industrial laundry press machine. He had not deenergized the machine prior to clearing it. When the machine was cleared, the lower portion completed its cycle by raising the stainless steel basket that Employee #1 was laying on. Employee #1 was crushed between the basket and the upper portion of the laundry press. Employee #1 was pronounced dead at the scene.
