Investigation Summary
On July 19, 2012, an employee was working for an insurance company and travel agency in San Luis Obispo, CA. She was exiting a toilet stall in the bathroom to wash her hands, when she slipped and fell. She hit her head and also sustained injuries to her hip and leg.
