Investigation Summary
At approximately 4:30 a.m. on July 13, 2012, Employee #1 was attempting to make repairs to a juice tank. The pump on the bottom of the tank in Building #1 quit pumping and he went to remove the hose to unplug the line. When Employee #1 removed the hose, hot apple pieces and juice sprayed on various parts of his body and he received second degree burns of the face, neck, torso, and left upper extremity. The pump was a Waukesha pump, s/n: 13960-SS, with 3 inch inlet and a 2 inch outlet with a 2 inch shutoff. The two inch hose was 25 feet in length, and the 3 inch hose with 8 feet in length.
