Investigation Summary
On January 13, 2012, around 10:22 PM, a Meat Cutter suffered an amputated right thumb due to an unguarded meat cutting saw. At the time of the accident, Employee #1 was performing a regularly assigned task of cutting cow knee bones, and meat in the attempt to prepare the bones for sale. Employee #1 was cutting the bones using a meat saw. According to a security camera video, Employee #1 passed his hands through the saw blade without any of the bones or meat materials in his hands, as a result Employee #1's right thumb was amputated. Employee #1 was taken to a local hospital where he stayed for several days following the accident.
