Investigation Summary

On September 2, 2011 Employee #1 of Eastland Corporation DBA C& H Meat Company was working as a meat cutter when a meat cutting table fell over. Employee #1 sustained a fractured right leg that required surgery resulting in hospitalization in excess of 24 hours after the table fell over onto her leg. There was a surveillance video that was reviewed and at the time of the accident Employee #2 and Employee #3 were removing piles of cut ribs from one end of the table while Employee #1 was trimming ribs at the other end. When Employee #2 removed the last pile of ribs, the table can be seen beginning to slowly tip toward Employee #1, falling over onto her leg. Interviews of the witnesses and the injured worker do not reveal that the table had incurred damage or had been repaired or altered prior to the inspection, Inspection of the table did not reveal any recent evidence of modifications or alterations or repairs. The floor was also examined and was shown to be level.
