Investigation Summary
At approximately 11:05 a.m. on May 5, 2011, Employee #1, a yard worker for Commercial Sand Blast Company, assisted a forklift operator/supervisor to store an approximate 30,000 pound 60 ft long cell tower pipe onto two metal horse racks. Employee #1 placed angles on the left end of the cell tower pipe. He proceeded to crouch down and place angles on the right side, in order to keep the cell phone tower pipe from rolling. The forklift driver/supervisor lowered the cell tower pipe onto the two horse racks, which stored another cell tower pipe, and it began to roll. As a result, this caused both horse racks to collapse. One of the horse racks, adjacent to Employee #1, inadvertently struck his head and arm, which caused unspecified fractures and resulted in his death.
