Employee's Fingers Are Amputated in Meat Slicer

On November 11, 2010, Employee #1, of Burnett and Son Meat Packing, was operating an Anco slicer (meat slicer). Employee #1 noticed the slicer was producing nonuniformed meat slices. He brought the problem to his Supervisor's attention and requested that the slicer be fixed. Employee #1 had become frustrated after multiple requests to get the slicer fixed were ignored. He asked his Supervisor to come over to the slicer to demonstrate what he thought was the problem with the machine. The Supervisor watched Employee #1 touch the depressor in order to show how loose it was. At that moment, Employee's #1 right hand slipped into the rotating blades, which resulted in the amputation of his middle, ring, and pinky fingers. Employee #1 was administered first aid and then was taken to Arcadia Methodist Hospital for treatment. The Division determined Employee #1's injuries were caused by trouble shooting when not implementing the machine lock out/tag out procedures. A general citation was issued to the secondary employer for not ensuring the meat slicer was deenergized, and the power source positively locked out prior to allowing their employees to engage in any repair to the machine.
