Investigation Summary
At approximately 7:20 a.m. on November 8, 2010, Employee #1, who had been working for one week at a company that manufactured wooden pallets and employed 25 workers, was performing duties of a general laborer, including unloading material from a machine. For 20 minutes, Employee #1 had been performing a first-time assigned task of removing wooden material from a double notching machine to assist the machine operator in producing pallet stringers. Employee #1 was caught in the double notching machine and sustained multiple amputations, apparently of both arms. Employee #1 was transported and admitted to Saint Francis Medical Center for surgery and was hospitalized for more than 30 days. On November 8, 2010, the Division was notified about the accident by the fire department at 7:49 a.m. and by the employer at 8:30 a.m. The investigation concluded that the double notching machine's point of operation was not guarded, and the Division issued the employer a serious accident-Related citation.
